Job Description

Carers ACT is the recognised, expert voice with, and for carers while being the major provider of carer services and supports in the ACT and Southern Highlands Region. Our dedicated and skilled staff play an essential role in developing the organisation's goals and priorities to meet our mission.

About the Role

We are currently seeking an Aboriginal and/or Torres Strait Islander person in the role of a Care Planning Coordinator.  The role is responsible for providing high quality support to Aboriginal and Torres Strait peoples who provide unpaid care to people living with disability, chronic health or mental health conditions or are frail aged. This role is available as either a full time or part time contract role until 30 November 2019, with the possibility of extension dependent on funding. Duties of this role include:

  • Providing information on Carers ACT services, as well as basic information on external providers to carers;
  • Working with Aboriginal and Torres Strait Islander carers to identify supports required to maintain the caring role. Develop a care plan to identify the actions, supports and resources required;
  • Regular engagement with Aboriginal and Torres Strait Islander carers to ensure progress against the implementation of the plan;
  • Plan, implement and coordinate suitable group activities that provide carers with the opportunity to connect with other carers and have a break from their caring role;
  • Participating in the Carers ACT Reconciliation Action Plan working group;
  • Undertaking outreach activities across Southern NSW;
  • Maintaining accurate records of all contacts on Carers ACT database;
  • Working with Aboriginal and Torres Strait Islander carers to empower and support them to navigate the care system;

 Skills and Experience

  • Qualifications and/or experience in health, mental health, aged care or disability fields;
  • Experience and/or qualifications in the delivery of care planning, case management or intake and assessment processes;
  • Demonstrated empathetic and flexible approach to manage the sensitivities of carer issues;
  • Understanding of issues experienced by unpaid family carers;
  • Ability to build and nurture relationships and networks with service providers and target groups within the ACT community;
  • Well-developed organisational, communication and interpersonal skills;
  • Good computer skills including proficiency in Microsoft Office suite and use of databases;
  • Good client documentation, record keeping and file management skills with an attention to detail;
  • Current driver's license;
  • Current Working with Vulnerable People registration and National Police Check, or the ability to obtain.
