Job Description

Indigenous Health Project Officer

 

Part-Time Position 0.8 FTE

 

MACCHS are seeking an Indigenous Health Project Officer to join our Integrated Team Care Program. The Indigenous Health Project Officer (IHPO) role provides leadership on Indigenous health issues. The position is funded by CSAPHN as part of the Integrated Team Care (ITC) program. The IHPO role builds on existing partnerships to develop and implement strategies to improve the capacity of mainstream primary care providers to deliver culturally appropriate primary care services to Aboriginal and Torres Strait Islander people of the Adelaide Hills, Southern Fleureiu and Kangaroo Island region.
 

The IHPO role sits within the chronic condition team and the successful applicant will work closely with GP practices to facilitate timely, culturally appropriate and holistic primary health care to clients registered with the ITC program.
